OneBridge has returned to partner with Coast2Bay, providing free health outreach to 12 priority unit complexes weekly, from August to mid November.
In April/May, OneBridge piloted this service with Coast2Bay over four weeks, with the following results:
- 309 engaged with the service
- 97 clinical consultations
- 65% of tenants returned for ongoing consultations
In the first 2 weeks of onsite skin cancer checks, nurses diagnosed five cancers.
In just the first 4 weeks the team have accomplished:
- 10 escalations to a specialist nurse practitioner via telehealth
- Providing suicide prevention and grief support at clinics
- 17 incidences of wound care and supplies handed out
- Supporting and motivating people to return to their everyday activities by education and guidance through anxiety
- Providing and paying for antibiotics to individuals with wound infections.
- SMART goals: time rebuilding confidence to reapply for work
- Medication monitoring
- 18% of consultations required referrals for support: MAC, NDIS, Centrelink, dentists, mental health, and drug and alcohol
- Working with local agencies, Hospital and Health Services, & social housing groups for optimal individual care
The OneBridge program is expanding nationally and offers vulnerable people equitable, accessible health care.
